After you submit this form, we will send you an intake packet and payment information to complete your enrollment. Your spot in the group is reserved once all required paperwork and payment information have been received. Group fees are $35 per session and are paid directly to Crosscare Institute prior to or at the time of each session. If you need support with scheduling or payments, we’re happy to work with you to find an option that feels manageable.
